Common Solutions llc provides a multitude of services to help support those looking to navigate their way through the maze of health challenges. See below for a detailed list of services provided by the Common Solutions team.
Behavorial Health
Develop Behavioral Support Plans- Using the FAST assessment tool, observations both in the home and community, to develop a behavioral support plan to help navigate family members, staff and others in decreasing/eliminating behaviors.
Residential Services
- To secure and maintain residential living supports
- Providing up to 24hr services and supports which are designed to ensure the health, safety and welfare of individuals with special needs. To assist in the acquisition, improvement, and retention of skills necessary to support individuals to become more independent. Theses skills include assistance in ADL’s, dinning needs, medication management, financial needs such as paying bills and budgeting.
- Medication Management: Develop and review Titration Plans, and review and recommend Psychotropic Medications.
The Community Integration and Habilitation (CHIO)
The Community Integration and Habilitation (CHIO) waiver strives to enable individuals to gain access in their community or community-based settings. The community integration and habilitation waiver provides services that enables individuals to integrate into their communities. This service provides skills necessary to secure volunteer opportunities, gainful employment, and or other person-centered activities while promoting independence in ones community. Community-based services (HCBS) through the Community Habilitation waiver allows flexibility in providing the supports necessary to help individuals gain and maintain optimum levels of self-determination and community integration.
Respite Services
Respite services are those services provided in temporary and periodic reprieve for the caregiver. Services may be in the individuals home, in the private home of the caregiver, in an adult foster care home, or in a Medicaid certified nursing facility.
Transportation
Patients with an Indiana Medicaid waiver are eligible for Common Solutions Transportation services. We aim to navigate you to your destination in a timely fashion with attentive client care.
Participant Assistant Care (PAC)
Common Solutions offers exceptional Participant Assistance and Care (PAC) services to provide support for daily activities. Common Solutions PAC services include assistance with paying bills, social skills, household chores, meal prep, and daily hygiene. The goal is that each participant will become more independent and learn daily life skills. We use hands-on and informative methods catered to each participant.
